Polk County Middle School students in grades 7-8 are eligible to participate in sports.

You must have a current sports physical before you can participate in a sport. Sports physicals are good for one year. In addition, parents and athletes must complete a concussion information form before participating. Concussion forms and sports physical forms are available at the school office.
