Opioid Settlement Advisory Committee launches survey for Polk County residents

POLK COUNTY—The Opioid Settlement Advisory Committee is launching a community survey designed to understand substance misuse across Polk County. 

The input from residents will be instrumental in guiding the effective allocation of funds derived from the Opioid Settlement, ensuring that community needs are met in the effort to combat substance use. Adults aged 18 and over are encouraged to contribute their insights to this important initiative. The committee seeks to gather diverse perspectives and information that will inform planning and resources dedicated to addressing substance misuse in the region.

Residents can participate in the survey through three convenient methods. They can choose to complete it online by visiting https://nccu.co1.qualtrics.com/ife/form/SV_ePOXxkWGAzpHfuu.

Alternatively, those who prefer a physical copy can request a paper survey by contacting the Polk Health and Human Services Agency at (828) 899-0843.

To incentivize participation, the first 400 respondents will receive a $35 gift card as a token of appreciation for their time and feedback. There is a limit of two gift cards per household.
