Polk County puts out survey to assess the community’s health

POLK COUNTY—Polk County officials are asking for the community’s opinion. 

The county has put out a survey to assess the community’s health. 

Officials are gathering data so that future decisions can be made as to how best care for the residents of Polk County. 

Polk residents are encouraged to take the survey so county officials can better determine where and how money and efforts should be spent. 

The survey takes about 10 minutes to complete. 

The survey includes several questions, including people’s age, possible health and mental health issues, smoking and drinking habits, how much people exercise, if people have access to food and housing, have ever been discriminated against because of race and have adequate medical care.
