Due to numerous complaints regarding ongoing issues in Polk County with Verizon Wireless, Polk County Local Government is asking citizens to provide information to help Verizon target the troubled areas.

Verizon Wireless engineers have agreed to review feedback from Polk County residents about service issues in the area.

Any Polk County resident who has experienced a service issue within the county boundaries is asked to complete the form linked below. Once the data is collected, it will be submitted to Verizon engineers for review.
